Our assignment forms are in 3 parts.
The ﬁrst part of the form is the assignment. This is the part of the form which assigns the beneﬁts from the beneficiary to the funeral home.
The 2nd part of the form is the power of attorney to our ﬁrm. This allows us to act on the beneﬁciaries behalf to complete the claim for payment.
The last part, is the reassignment from our client to our ﬁrm. This indicates to the insurance company to send the assigned amount of the proceeds to the Ford Acceptance Corporation.
