Our assignment forms are in 3 parts.

The first part of the form is the assignment. This is the part of the form which assigns the benefits from the beneficiary to the funeral home.

The 2nd part of the form is the power of attorney to our firm. This allows us to act on the beneficiaries behalf to complete the claim for payment.

The last part, is the reassignment from our client to our firm. This indicates to the insurance company to send the assigned amount of the proceeds to the Ford Acceptance Corporation.
